I understand that because of my employment at Case Western Reserve University, I have access to datasets containing individually identifiable information, which are prohibited by the Family Educational Rights and Privacy Act of 1974 as amended. I recognize that student information from each source and in all forms is confidential and that I am only available for the performance of my official duties as a collaborator or box partner. I will protect the privacy and confidentiality of the information of the students I have access to and use it exclusively to carry out my official duties. I do not agree to access student information outside my responsibility unit. I also recognize that I fully understand that disclosing this information to unauthorized persons imposes sanctions on the university imposed by the U.S. Department of Health, Education and Welfare. Staff are also expected to be familiar with the university`s “Acceptable Use of Information and Technical Resources” directive, staff are also expected to be responsible for keeping passwords secret, and not to use another person`s password to access student information. University staff, whether employees, teachers, students, related companies or temporary staff, may have to access student data to perform employment tasks. It is important to understand that the Family Educational Rights and Privacy Act (FERPA) protects student data from illegal use and disclosure.
To further understand FERPA and what you will agree below, it is highly recommended that you follow both the FERPA University online training video and the university`s interpretation of FERPA. I also recognize that such deliberate disclosure or not is also contrary to Case Western Reserve University guidelines and may constitute grounds for disciplinary action, in accordance with the procedures established in the Faculty Manual (for Faculty members) or the General University Guidelines (for all others).
